Family Connect Program – Keeping Families in the Know

The Family Connect Program is a proprietary program developed by the Canton office of Visiting Angels, to ensure families are always connected to the care their loved one is receiving. This program is unique to Canton clients and offers flexibility and easy access to important information for family and doctors with one click of opening an email.
Visiting Angels places a tablet in every client's home for our Care Professionals to use. On the tablet they have the client's care plan. The care plan is information about the client that is vital to how a caregiver specifically provides care to that person. Not only do our Care Professionals and families have digital access to the care plan but there is also a feature for injury reports. This is something a paper care plan would not traditionally offer but is vital to making sure our clients are safe and to help prevent future injuries. The injury reports have camera access to capture the injury to the client, should one happen. It contains all the forms necessary to document any injuries correctly.
What makes this program even more valuable is the opportunity to make detailed documentation. All visit notes are customized to the client care plan and are extremely in-depth. At the bottom, our Care Professional can add notes after each shift.
The caregiver then signs the tablet and the notes are sent, via encrypted email, to the office staff for review and to any and all family members. This happens in real-time, so the staff and families know what’s going on as it’s happening. The Family Connect Program has been used to communicate with hospice agencies and the client’s doctors as well. Since implementing this new program clients and families have reported loving the new and efficient communication.
