Senior Life Care Navigation

At Visiting Angels®, we view senior care as a journey for your loved one. Over the years, as your loved one's care needs change, this journey can head in unplanned directions. With Life Care Navigation™, we quickly adapt to these changes by adjusting your loved one's care plan. This way, you don't need to worry about unexpected twists in your loved one's care.

Life Care Navigation covers a wide range of services from basic programs such as companion care and personal care to specialized services, including our Safe & Steady Fall Prevention Program. Your loved one's initial care plan can include as few or as many of these services as needed.

Over time, we customize this care plan by adjusting the existing services or adding new ones. As a result, your loved one will have a personalized care plan every step of the way.

Types of Senior Life Care

  • Alzheimer's Care
Visiting Angels provides skilled and compassionate Alzheimer's care in the comfort of our clients' homes. With our help, your loved one can enjoy the comfort of a familiar space and maintain a sense of independence. As Alzheimer's progresses, we adjust our services to maintain quality of life and help our clients age in place for as long as possible.
  • Companion Care
Companion care is one of our most popular in-home care services. This type of senior care is focused on day-to-day tasks and activities your loved one may have trouble performing on their own. Services include light housekeeping, meal planning and meal preparation, and accompaniment to doctors' appointments.
  • Dementia Care
With Visiting Angels, memory care isn't limited to Alzheimer's disease. Our dementia care services include specialized care for clients with less common types of dementia. If you have a loved one who's living with dementia, we can help them live at home safely and comfortably.
  • End of Life Care
When a loved one is nearing end of life, Visiting Angels acts as a support system for the entire family. Our caregivers can help make your loved one more comfortable throughout this process. At the same time, we reduce the burdens placed on your family so they can spend more time with your loved one.
  • Palliative Care
Our palliative care program takes a holistic approach to in-home senior care. This comprehensive caregiving approach focuses on physical health and on the mental, emotional, and spiritual well-being of our care recipients.
  • Personal Care
Our personal care services are an ideal choice for seniors living with limited mobility, a physical disability, or a chronic health condition. This type of senior care includes assistance with activities of daily living—such as dressing, grooming, and bathing—and other skilled caregiving duties.
  • Ready-Set-Go Home Transitional Care
Transitioning home after a stay at the hospital can be difficult for seniors. It can also put them at significant risk of medical complications. Ready-Set-Go Home is a transitional care program designed to reduce these risks and make the transition home easier on seniors and families.
  • Safe & Steady Fall Prevention Program
Our Safe & Steady fall prevention program can reduce your loved one's risk of falling at home. Fall-related injuries are the number one reason why seniors lose their independence. Thankfully, most slips, trips, and falls can be avoided with the right precautions.
  • Social Care Program
Does your loved one need help with digital technologies? Our Social Care program can be a valuable resource. This program helps clients navigate new and connected technologies to connect with family and friends through video calls, social media, and smart devices.
