How Transitional Care in Baltimore East Reduces Hospital Readmissions

Ready-Set-Go Home with Transitional Care

Did you know there's a way to increase your loved one's chances of making a successful transition back to life at home after a hospital stay? Professional transitional care is available for seniors who are being discharged from the hospital. At Visiting Angels® Baltimore East, we have a program called Ready-Set-Go Home that's a great fit for elderly adults who need a helping hand during the recovery process.

What Causes Hospital Readmissions in the Baltimore East Area?

While there are many causes of hospital readmissions, one common issue among seniors is a lack of energy or mobility that leads to an inability to follow instructions from physicians. After being ill or injured, many elderly adults find it difficult to complete basic routines that once were easy. When they struggle to keep track of their medications, prepare healthy meals, and bathe, their health can suffer. As a result, nearly one out of five seniors on Medicare return to the hospital within a month of their discharge.

How Our Transitional Care Providers Can Help

Our care providers at Visiting Angels Baltimore East can assist your loved one in a wide variety of ways to prevent a decline that could lead to rehospitalization. As we believe preparation is one of the most important steps in transitional care, we start our Ready-Set-Go Home program at the hospital.

We'll guide you and your loved one through the discharge planning process and help outline what to expect during their transition back to life at home. We'll also give you our discharge planning guide for a resource you can always refer back to during your loved one's recovery.

When your loved one returns home, they will have compassionate support from a care professional who understands their unique needs and challenges. Our caregivers are able to assist with personal care routines such as bathing and grooming and can offer mobility support and medication reminders. They can also arrange safe transportation to doctors' offices to ensure your loved one attends crucial follow-up appointments as scheduled.
