With Ready-Set-Go Home, our transitional care program for seniors in the local area, elderly adults can access the personalized support they need to make a successful recovery following a hospital discharge.   2. In-Home Care. After your loved one is discharged, their caregiver will be by their side assisting with a wide range of daily activities at home. From medication reminders to support with bathing and grooming, our caregivers are ready to offer a helping hand with the tasks seniors often find challenging following a hospital stay.
  3. Support from Our Office. Our office staff are always working behind the scenes to ensure our caregivers are equipped to help their care recipients make a successful recovery. Our team utilizes innovative tools to identify high-risk seniors and can also facilitate deliveries and coordinate reminders.
