Ready-Set-Go Home with Transitional Care

Ready-Set-Go Home is a transitional care program from Visiting Angels® North Hills of Pittsburgh designed specifically for seniors undergoing a hospital discharge. The aim of this program is to help seniors make a successful transition home following their hospitalization. By providing compassionate care and assistance, we strive to reduce hospital readmissions for seniors living throughout the North Hills Pittsburgh area.

A staggering 20% of discharged Medicare patients return to the hospital within 30 days of leaving. As elderly adults have a significantly lower chance of making a recovery if they are re-hospitalized during this period, reducing the risk of readmission is extremely important.

  1. In-Hospital Support. We’ll begin caring for your loved one before they ever leave the hospital. This includes preparing them for the upcoming changes by providing detailed information about the discharge process.
  2. In-Home Support. Our care providers will ensure your loved one is safe and comfortable at home following their hospital discharge. In addition to assisting with basic activities of daily living, from bathing and grooming to meal preparation, we’ll also act as another pair of “eyes and ears” in the home. Our caregivers look out for risky behaviors that could lead to a readmission and alert family members when seniors deviate from proper recovery protocols.
