We developed our Ready-Set-Go Home program specifically to make sure that seniors get the support and assistance they need to follow their discharge directives and increase the odds of partial or full recovery at home.

Our program at Visiting Angels Corpus Christi features a three-prong approach to transitional care:

  • At the Hospital - Before being discharged, we can visit at the hospital and guide you through the process of discharge planning based on your doctor's instructions. We'll also provide a guide that includes questions you can ask the doctors and nurses, general details about the discharge process, and tips on lowering your chances of readmission.
  • At Home - Once a loved one has left the hospital, our caregivers will be there to offer skilled non-medical aid. We can also monitor our client's condition, keeping an eye out for changes in their cognitive or physical condition and obstacles to their recovery. If members of your family are acting as caregivers during this time, we'll do what we can to support you in this role.   
  • At Our Office - In addition to the caregivers we send to your home, our office staff at Visiting Angels Corpus Christi also play a role in your transitional care. Our staff can identify clients with a high risk of readmission and coordinate food and medication deliveries, escorts to appointments, and important reminders.
