Ready-Set-Go Home is our comprehensive transitional care program for elderly adults in the Long Beach area who could use a little support with an upcoming hospital discharge. Why is Transitional Care Important?

After a hospital stay, seniors lack energy, feel weak, and lose mental clarity. Sometimes, they need some external support to do important things like drink fluids regularly, follow detailed medication schedules, bathe, and prepare healthy meals.

We give care recipients and their family members our Ready-Set-Go Home discharge guide to help them prepare for the return home, coordinate care, and communicate with health care providers. We also take the time to visit care recipients in the hospital and make sure they have a detailed plan in place for the upcoming transition.

Upon seniors’ return home, our caregivers help out in various ways. They can act as an extra pair of eyes and ears to monitor the senior and provide medication reminders. They can also cook nutritious meals and snacks and assist with physically challenging tasks like bathing and grooming. We create personalized transitional care plans for each senior to make certain the care they receive reflects the activities they have most difficulty completing on their own.
