Hospital visits aren't easy on seniors and their family members, especially when the risk of rehospitalization following discharge is so high. Without external assistance, seniors may not be able to properly look after their own well-being, a situation that typically ends with rehospitalization.

How Our Transitional Care Program Can Help Seniors in Reno

Ready-Set-Go Home utilizes a three-pronged approach to transitional care that aims to cover all of seniors' needs, facilitate a smooth recovery, and reduce rehospitalization rates among elderly adults. The first prong of this care process includes help at the hospital. Proper discharge planning is crucial for a successful recovery, and our care coordinators work alongside families to assist with the discharge process. As families gather information from physicians about their loved one's condition and instructions for their care following discharge, we create personally tailored care plans including the services seniors are most likely to require.

The second prong involves home care visits. We match care recipients with professionals who can offer the helping hand they need to remain safe and comfortable throughout their recovery. Caregivers can assist with tasks such as personal care activities, remembering medication schedules, cooking healthy meals, and more.

The third prong of care is conducted at our local office. Our staff members diligently work to coordinate care visits, deliveries, and reminders, helping ensure both seniors and caregivers have everything they need to increase the chances of a successful recovery.
