How Transitional Care in Providence Reduces Hospital Readmissions

Ready-Set-Go Home with Transitional Care in Providence

Ready-Set-Go Home is a transitional care program from Visiting Angels® Rhode Island designed to help seniors in the Providence area make a successful transition home after a stay at the hospital. This program is comprised of a three-step approach to reducing the need for readmission, which is a major risk for seniors in the first month following a hospital discharge.

We’re committed to reducing hospital readmissions for elderly adults in Providence and nearby communities such as Cranston, Warwick, Pawtucket, East Providence, Cumberland, North Providence, Newport, Bristol, Lincoln, Portsmouth, and Middletown. Through high-quality at-home care, we’ll assist your loved one during this critical time and help them ease back into life at home.

Why is Transitional Care in Providence Necessary?

Almost one-fifth of seniors on Medicare are readmitted to the hospital within thirty days of being discharged. These individuals have a much lower likelihood of making a full recovery if they are re-hospitalized during this one-month window, which is why reducing the chance of readmission is so important.

For many seniors who live alone, even the most basic tasks can be a challenge as they recover from an injury or serious illness. With specialized transitional care services delivered by a professional caregiver, elderly adults in the Providence area can have the support they need to follow doctors’ instructions and avoid risky behaviors.

How We Help Seniors in Providence Avoid Hospital Readmissions

To help increase the odds of recovery, we deliver customized care tailored to the specific needs of each of our clients. Here’s how our three-step transitional care program for the Providence area works:

  1. In-Hospital Care.  For best results, we begin the care process while the senior is still in the hospital. Our caregivers are available to provide support for seniors and their families and can help with preparations for the discharge process.
  2. In-Home Care. When the senior returns home, our caregivers will step in to offer assistance with basic tasks while helping the senior remain safe and comfortable during this crucial period. At Visiting Angels Rhode Island, we also monitor the senior for changes in well-being or behaviors that could put them at risk for readmission.
  3. In-Office Support. Our care coordinators work behind the scenes at our office to ensure caregivers have the full support they need. From medication and appointment reminders to assistance with coordinating deliveries, we’ll stay on top of important tasks to help promote your loved one’s recovery.
