Ready-Set-Go Home with Transitional Care in Providence

Ready-Set-Go Home is a transitional care program from Visiting Angels® Rhode Island designed to help seniors in the Providence area make a successful transition home after a stay at the hospital. This program is comprised of a three-step approach to reducing the need for readmission, which is a major risk for seniors in the first month following a hospital discharge.
