Reducing Hospital Readmissions for Seniors in West Atlanta, GA

20% of seniors with Medicare end up back in the hospital within a month of being sent home. If a senior is readmitted during this month-long period, they have a much lower chance of recovery. Visiting Angels of West Atlanta’s Ready-Set-Go Home program helps prevent hospital readmissions and smoothly transition into their regular life. Within this program, our team has developed a three-prong approach to give our clients the best chance of success.

Ready-Set-Go Home’s Three-Prong Approach

Research shows that preventable re-hospitalizations happen because clients don’t stick to their doctor’s orders. But, after a recent hospitalization, these orders can become difficult to stick to. That’s why our Visiting Angels are here to support our West Atlanta clients during this difficult time.

During our three-prong approach, we provide quality at-home care that helps seniors stick to their doctor’s orders. Our specialized at-home care takes into account any unique health and wellness needs that they may have.

• At the Hospital: A Visiting Angel will guide your loved one, and your surrounding family, through the discharge process. We will also provide a Ready-Set-Go Home guide that includes key information about having a successful hospital discharge, including important questions to ask doctors and nurses before you head back home.
• At the Client’s Home: Our caregivers know the common reasons why seniors skip follow-up appointments or medication doses. If your loved one exhibits those behaviors, our caregivers will step in to correct them. They will also keep a watchful eye for any changes to your loved one’s physical or cognitive well-being.
• At the Visiting Angels Office: Back at the office, our West Atlanta team will be available to support your family. We can accompany your loved one to appointments, coordinate medication deliveries, and more.
