You’ve conducted your research into home care agencies and you are ready to take the next step. Whether your loved one is still in a home, or they are in a facility and just need supplemental care – Visiting Angels can help. We provide non-medical assistance with activities of daily living (bathing, dressing, toileting, transfer, eating), personal care (incontinence, meal prep), light housekeeping, and transportation (errands, to appointments). Whether your loved one only needs help with a couple of tasks or need full care – we can help.

Our Process

The process is actually simple!

• Call the local office at 408-846-2988 and the Executive Director will have a chat with you to determine if indeed a partnership of care should be formed. During this time, we can give you a rough estimate of cost after we acquire basic information. The conversation should also provide you with a good feel for how we work, etc.
• If you would like to move forward after the initial conversation over the phone, a free consultation visit will be scheduled. During the visit – we will cover a wide range of topics, including but not limited to the following:
  • Client’s basic personal info
  • Client’s physical and mental status, and apparent needs
  • Fall risk assessment
  • Daily routines (including but not limited to meals)
  • What the family thinks is needed in terms of light housekeeping
  • Logistical details (home access, emergency procedures, etc.)
  • Articulate Care Plan goals and discussion
  • A site inspection for safety will be conducted and recommendations discussed
  • We will provide a packet of forms for the family to fill out. These include forms to acquire more details (does the client have a DNR, POLST, Advanced Directive; if hospitalized within the last two years – why, etc.).
• When you are ready to commit, we will need the packet of forms filled and signed, with a two-week deposit.
• While forms are being completed, we will identify an appropriate caregiver and discuss the caregiver’s background with the family. (Our selection process takes into account the client’s needs, the client’s personality and preferences, the goals and the preferred schedule. While we have enjoyed great success in matching a caregiver to a client, please know that if there is an issue that cannot be corrected, we will always provide you with the option of requesting an alternate caregiver).
• A start date is set.
• A first-week follow-up call is conducted to find out how things are going, if we need to make any adjustments.
• Follow-up calls are conducted for the duration of the service.
