If a discharge planner at Bryan East or a social worker at CHI Health St. Elizabeth told you your mother needs "private duty" help at home, you probably left the hospital with more questions than answers. Private duty home care is non-medical support paid for privately, extended shifts, overnight coverage, or full 24-hour care in the home, for a family that needs more help than a few hourly visits can cover. We also refer to it as "Personal Care". It's not Medicare home health, and it's not a nursing home.

This personal care page explains what "private duty care" is, when families in Lincoln use it, and how to get started supporting your loved one.

Private Duty Home Care, in Plain Terms

What it is: A trained caregiver privately hired through our Lincoln office who provides one-on-one personal care for your loved one in their home, for long shifts (8, 10, 12 hours), overnights, or around the clock.

What it is not: A nurse, a physical therapist, or a Medicare-covered service. Private duty care does not include wound care, injections, IV therapy, or anything that legally requires a licensed clinician.

How it's paid: Out of pocket, through long-term care insurance, or through VA Aid & Attendance benefits for eligible veterans and surviving spouses. Medicare does not cover it. Medicaid only covers it through specific Nebraska waiver programs.

Who provides it: Our Lincoln caregivers, background-checked, trained in transfers, medication reminders, dementia support, and hospital-discharge protocols. Our office provides backup coverage if your regular caregiver is out.

When Lincoln Families Use Private Duty Care

These are the four situations where families call us specifically for private duty coverage rather than standard hourly home care.

  1. After a Surgery at Bryan East or St. Elizabeth

Your dad just had a hip replacement. Medicare home health sends a PT three times a week for 45 minutes. The other 165 hours of the week, he needs help getting out of the recliner, to the bathroom, and staying off that hip. A 12-hour private-duty shift for the first two weeks home keeps him safe without anyone in the family having to take unpaid leave.

  1. Long-Duration Alzheimer's & Dementia Coverage

Your mom has moderate Alzheimer's and can't be left alone. Moving her into memory care at Legacy or Gateway Senior Living isn't the answer your family wants yet. Private duty lets her stay in her own house with a caregiver who knows her routines. Overnight shifts or full 24-hour coverage with live-in arrangements are both options.

  1. Overnight Safety After a Fall

Your father fell in the hallway at 2 a.m. last month. He's recovered physically, but he's unsteady and you haven't slept well since. A 10-hour overnight shift, 9 p.m. to 7 a.m., means someone is there if he gets up. You get your nights back.

  1. End-of-Life Support Alongside Hospice

Hospice from Tabitha or a local provider sends nurses and social workers on a visiting schedule. Private duty care fills the hours in between. Someone bedside so the family can rest, coordinate with hospice, and spend unhurried time with their loved one.
