Transitional Care with Visiting Angels
Every year, more than 13.2 million seniors are hospitalized in the United States. According to a study from the National Institutes of Health, about 5 percent of seniors return to the hospital within 30 days of their release. While 5 percent is not a significantly high number, what is shocking is that the same study found that 41 percent of those readmissions could have been avoided.
The reasons for hospital readmission can vary greatly depending on the individual. Yet, Visiting Angels is working to reduce this number with transitional care through our Ready-Set-Go Home program.
What is Transitional Care?
Transitional care with Visiting Angels is when a professional caregiver helps a client return home after a hospital stay. Like all of our care services, transitional care will look different for each client but may include:
- Providing transportation home from the hospital after a client’s stay
- Assisting with household tasks like housekeeping, cooking, and laundry so clients can rest
- Reminding clients to take their medications and complete other follow-up care instructions
- Providing transportation to follow-up appointments
- Assisting with personal grooming, showering, mobility, and toileting
Benefits of Transitional Care
The primary goal of our transitional care services is to reduce the risk of readmission for our clients. However, lowering hospital readmissions is not the only benefit of hiring a professional caregiver after your hospital stay.
- Help with recovery care logistics: When a senior leaves the hospital after a stay, there are often a lot of follow-up instructions, medication directions, and other recovery logistics that can feel overwhelming, especially if you’re not in your best health. Caregivers can meet with doctors and nurses and gather recovery information so it’s not solely on the client.
- Identify at-risk factors for readmission: The Visiting Angels office has tools to help caregivers identify if clients are at risk for readmission. If a client is at risk for readmission, caregivers can help clients find the help they need to keep them safe and healthy.
- Ease the stress of our client’s loved ones: It’s not always possible for clients’ family and friends to be by their side during the recovery process. Having a professional caregiver present means loved ones can feel at ease knowing someone is there to care for the senior in their life.
