They need outside help during this time and Visiting Angels can provide the care that they need to overcome their injury.

To reduce hospital readmissions, Visiting Angels has developed this three-pronged approach for our program:

  1. At the Hospital: Our caregivers can help you through the discharge process and provide valuable information on discharge planning. We also offer a “Ready-Set-Go Home” guide that gives detailed information on questions to ask doctors and nurses, hospital discharges in general, and tips on reducing readmission risks.
  2. At the Client’s Home: To ensure your loved one’s safety, we provide skilled, personal home care at their home. We know the reasons why many seniors skip appointments and don’t follow doctor’s orders. We stop those behaviors to give a client the highest chance of recovery.
  3. At the Visiting Angels Office: At our offices, we will be working behind the scenes to provide your loved one with the personal home care that they need to thrive. The staff at Visiting Angels can provide medication reminders, transportation to appointments, and help you coordinate food and medication deliveries.
