How Do You Help Us Prepare for Hospital Discharge?

As part of Transitional Care, we'll give your family our informative discharge guide to assist you with the preparation process. Then, we'll use your input to create a personalized plan of home care tailored to the needs and challenges your loved one is expected to have upon their return home.

Once your loved one is home, our trained caregiver can remind him or her to take medications, cook and/or assist in feeding them well-balanced meals, help with bathing, grooming and help with walking or transferring from bed to wheelchair.
