Our care coordinator works with clients and their families to develop a customized care plan to meet their needs. All client care plans are different and can change if additional care is needed. We also get to know each client before services start to match them with one of our caregivers. Our goal is to match clients with caregivers who can perform the proper services and have personalities that mesh well with the client.
