Our Three-Prong Approach

Our Ready-Set-Go Home program is designed to increase the chance of a full or partial recovery after being released from the hospital. We’ve learned that the vast majority of hospital readmissions happen because an individual doesn’t follow doctor’s orders. But, following a serious injury or illness, it can be very hard for an individual to follow certain tasks, even if they are basic. They need outside help during this time, and Visiting Angels can provide transitional care.

            To reduce hospital readmissions, Visiting Angels has developed this three-prong approach for our program:

  1. At the Hospital: Our caregivers can help you through the discharge process and provide valuable information on discharge planning. We also offer a “Ready-Set-Go Home” guide that gives detailed information on questions to ask doctors and nurses, hospital discharges in general, and tips on reducing readmission risks.
  2. At the Client’s Home: To ensure your loved one’s safety, we provide skilled, nonmedical support at their home. We know the reasons why many seniors skip appointments and don’t follow doctor’s orders. We stop those behaviors to give a client the highest chance of recovery.
  3. At the Visiting Angels Office: At our offices, we will be working tirelessly to provide your loved one with the support that they need to recover. The staff at Visiting Angels can provide medication reminders, transportation to appointments, and help you coordinate food and medication deliveries.
