  1. In-Hospital Care. We begin offering support for seniors and their families while the senior is still hospitalized. That way, we can facilitate a smoother transition for all parties involved. We’ll assist your family with planning the discharge process and provide valuable information, including our detailed “Ready-Set-Go Home” guide.
  2. In-Home Care. Once your loved one is safely at home, we’ll assist them with basic activities of daily living and provide the physical and emotional support they need during this sensitive time. While helping them follow through on their doctors’ instructions, we’ll also act as an extra pair of “eyes and ears” in the home, alerting family caregivers to any changes in the senior’s well-being.
  3. Behind-the-Scenes Support. Our office staff will be working tirelessly to facilitate a safe recovery for your loved one. In addition to utilizing innovative tools for identifying high-risk clients, our office also coordinates reminders and deliveries.
