How Transitional Care in Asheville Reduces Hospital Readmissions
Ready-Set-Go Home with Transitional Care
Transitional care can be a crucial asset for seniors who have experienced an extended hospital stay and are about to make a return home. At Visiting Angels® Asheville, we offer a program called Ready-Set-Go Home. Through this program, we give elderly adults the various forms of support they require to adhere to doctors’ orders and make a successful recovery.
Understanding the Importance of Transitional Care in Asheville
Elderly adults have an especially high risk of encountering issues that could lead to a hospital readmission in the 30 days following their initial discharge. When they are re-hospitalized during this time period, they have a much lower chance of making a full recovery. For this reason, reducing the occurrence of hospital readmissions in this age group is crucial.
With our help, you can ensure your loved one has the assistance they need following their hospital stay. We connect care recipients with professional caregivers who are ready to provide essential personal home care services tailored to the unique needs of each individual. When you partner with us, you can have peace of mind about your loved one’s well-being during those critical first weeks home.
About Our Approach to Transitional Care
At Visiting Angels Asheville, we’ve implemented a three-prong approach to transitional care that effectively reduces hospital readmissions for our care recipients. Here are the steps we take through our Ready-Set-Go Home program:
- In-Hospital Care. We begin offering support for seniors and their families while the senior is still hospitalized. That way, we can facilitate a smoother transition for all parties involved. We’ll assist your family with planning the discharge process and provide valuable information, including our detailed “Ready-Set-Go Home” guide.
- In-Home Care. Once your loved one is safely at home, we’ll assist them with basic activities of daily living and provide the physical and emotional support they need during this sensitive time. While helping them follow through on their doctors’ instructions, we’ll also act as an extra pair of “eyes and ears” in the home, alerting family caregivers to any changes in the senior’s well-being.
- Behind-the-Scenes Support. Our office staff will be working tirelessly to facilitate a safe recovery for your loved one. In addition to utilizing innovative tools for identifying high-risk clients, our office also coordinates reminders and deliveries.
