Our Three-Step Approach

Preventing readmission to a hospital is easy when you follow the doctor’s orders, but it’s not always simple for the elderly to do so. After an illness or injury, it can be almost impossible to take care of themselves while home alone. Therefore, outside support is a must during this 30-day post-discharge period.

Here’s how we reduce readmission:

At the Hospital. Assistance begins while a senior is still in the hospital. Our companions provide supportive services for our clients and their family members.

At the Client's Home. Aid is given to our clients after their discharge, ensuring that your loved one is comfortable and safe during their recovery. Throughout that time, our at home caregivers take note of everything that happens in the home, reporting on any changes in your loved one’s cognitive or physical health. Our care providers also provide respite care to family members who need a break from caregiving.

At the Office. Behind the scenes, our office support staff is behind our clients, their family members, and our at home caregivers. Aside from reducing hospital readmission, we can also accompany your senior to medical appointments, provide medication and appointment reminders, and help you coordinate medication, equipment, and food deliveries.
