Home care is personalized support that helps older adults remain safe, comfortable, and independent at home. At Visiting Angels East Metro (Minnesota-licensed Home Care provider), care is coordinated and overseen by a Registered Nurse (RN) and tailored to each client’s personal needs. Whether you’re looking for an in home caregiver for a few hours a week or more comprehensive daily support, we’re here to help.

RN-Directed Professional Services

• Care coordination & case management

• Medication management (set-ups, reminders, monitoring)

• RN assessment & care plan development

• Delegated/assigned tasks per RN direction (e.g., catheter or ostomy support, oxygen use guidance)

• Ongoing RN supervision and reassessment

Personal Care & Daily Living Support

• Bathing, grooming, toileting

• Safe transfers & mobility assistance

• Meal planning & preparation

• Support with exercise or home programs recommended by healthcare providers

Care Management 

An RN will evaluate needs, confirm what we can safely provide, and establish the right protocols—collaborating with your family and other providers. We also work alongside hospice providers when families need added support.
