Ready
First, your family will get started by working with Visiting Angels of Aurora to create a customized one-on-one elder care plan based on your loved one’s individual health needs, preferences, habits, and more. If your loved one requires any specialized care, we will make sure they are able to access it, even if it is needed around the clock.
Set
Transitional care begins in the hospital. Our Angel will meet your loved one and your family, and gathers information about aftercare, follow-up appointments, changes in medication, dietary concerns, and more. We will also make sure your loved one remains comfortable in the hospital and can provide companionship care during hours when family members are not available.
Go Home
Once your loved one has returned home our home health aides will make sure that they have everything they need to make a full recovery and avoid readmission. That includes following aftercare instructions, preparing healthy meals, encouraging rest and hydration, and being another pair of hands that can assist with any need your loved one might have.
